Formlane is a form builder you install on your own server. Build any form with a visual drag and drop builder, start from 31 ready-made forms, and put it on any website with one line of code or a link. Answers stay in your own database: read them, export them to CSV, get them by email and send them to Zapier, Make or n8n.
No monthly fee, no limit on forms, answers or users. Features- Visual builder: drag and drop fields, side-by-side fields, multi-step forms, undo and redo
- 17 field types: text, email, phone, URL, number, date, dropdown, radio, checkboxes, star rating, 0–10 scale (NPS), consent, file upload, hidden field, heading and text
- Conditional logic: show or hide fields depending on earlier answers, checked in the browser and on the server
- 31 ready-made forms in 7 groups, each with its own design
- Design without code: 12 looks, colors, local fonts, corners, field and button styles, logo, live preview on computer and phone
- Embed anywhere: one line of code (WordPress, Elementor, Webflow, plain HTML) with automatic height and UTM parameters, an iframe, or a hosted page
- Answers: search, date and unread filters, bulk actions, file downloads, views and answer rate per day, CSV export that opens cleanly in Excel
- Emails: notification to your team and confirmation to the person in the form’s language, with tags, through your own SMTP server
- Webhooks: signed JSON (HMAC SHA-256), automatic retries, delivery log
- Spam protection: honeypot, time check, rate limits, plus optional Cloudflare Turnstile, hCaptcha or reCAPTCHA
- Limits and privacy: close after a number of answers or on a date, optional IP storage, automatic deletion of old answers
- Team with roles (owner, admin, editor, viewer), two-factor sign-in, activity log
- Admin in English, French, Spanish, German, Portuguese and Tagalog
- Web installer, updates by uploading the new zip, backups of replaced files
- PHP 8.3 or newer
- MySQL 8, MariaDB 10.6+ or SQLite
- A cron job every minute, or a web cron (both explained in the docs)
